Permission to Render Medical Treatment

For each family member, the following permission form needs completed based on the members age. Adults are 18 years of age and over and minors on under 18.

Waiver and Release of Liability

For each family member, the following waiver needs completed based on the members age. Adults are 18 years of age and over and minors on under 18.

USA Water Ski Insurance

Each family must join USA Water Ski to be insured. Proof of insurance is required prior to participation. To obtain insurance go to USA Water Ski and complete the necessary forms. You can also join by calling 863-324-4341. When applying for insurance, be sure to indicate "NSS" under disciplines.
